Tooth colored fillings material has undergone continuous and tremendous changes in the fillings durability, esthetics and material handling. The most versatile and widely used tooth colored filling is composite resin filling. A composite is a material in which filler particles are encased in and bound together by a hard matrix material. Endodontics is the field of dentistry that deals with treating disease inside the tooth.

Endodontics is the field of dentistry that deals with treating disease inside the tooth. If a tooth has been badly decayed or traumatized the pulp inside the tooth can become infected. When toxins and bacteria leak out the openings at the tips of the root a dental abscess can form. In order to treat or prevent this dental abscess the dentist cleans out all the necrotic pulp tissue from within the pulp chamber and root canals through the required dental treatment.

Once the tooth is sufficiently cleaned out and as sterile as possible, the root canal system is sealed off. Bacteria are prevented from infecting the tooth again in most cases (a small percentage of root treatments do fail, especially if they are only done long after the abscess develops. However, many teeth are successfully saved from extraction, and root treatments are a valuable treatment option against teeth extractions)
Steps in completing a root canal treatment

Throughout a root canal treatment the dentist will need several x-ray pictures of the tooth to determine the position of the instruments and filling material inside the canals. The dentist also advises to have a good pre-operative picture to determine the approximate length of the roots and their particular shapes and curvatures
Access opening is done in the root canal treatment
First the dentist will remove any decayed and weak tooth structure. A suitable opening into tooth's pulp chamber is prepared to allow access to the openings of the root canals in the pulp chamber floor.

Instrumentation used for root canal treatment
The root canals are identified and then cleaned out with special root canal instruments. Some are used by hand, others rotate in a hand piece. They all have the function of removing debris from the canal and smoothing and shaping the walls of the canals to create a smooth, clean surface along the entire length of the tooth root canal. The canals are constantly rinsed to aid in debris removal and canal sterilization. In some, but not all root canal cases, the canals may be dressed with various medications and sealed with a temporary filling material to allow time for the infection to clear.

Sealing is done in root canals
Once the tooth root canals are suitably cleaned and shaped and infection free they can be sealed off. A root filling material (usually a rubbery material called gutta percha) is placed into each canal with a special sealer.

Final restoration becomes important in root canal process
Placing a suitable final restoration is almost as important as the root treatment itself. If the restoration leaks bacteria can migrate down the side of the root sealer and re-infect the tooth. A suitable permanent filling is placed into the access opening. Teeth with large amounts of damage may need a post (a metal or fiber reinforced resin rod) down one or more canals to anchor the filling securely onto the root of the tooth.

However the access opening is closed, root treated teeth will always be weaker than natural teeth and prone to cracking or splitting. A serious fracture could mean removing the tooth that was so carefully saved with the root canal treatment. A good way to ensure against such a disaster is to crown the tooth.

Risks and complications in the root canal treatment
Despite the best efforts of a dentist, a small percentage of root canal treatments do fail, especially if they are only done long after the abscess is developed in the tooth root. The failure rate increases if the root canal treatment is a repeat treatment of a previously failed root canal, or if some damage has occurred to the root during reaming. However, many teeth are successfully saved from extraction, and root treatments are a valuable treatment option.

We use very delicate instruments during treatment to clean and shape the tooth. Very occasionally these root canal instruments can separate (break) in the canals. This does not necessarily mean the entire root treatment is a failure. Most often the fractured piece can be removed or by-passed, and a normal root filling placed. Sometimes the root treatment is completed leaving the piece of instrument in the root. Most often this is not a problem. If the treatment does fail one can consider an alternate surgical solution: either extracting the tooth or doing an apicoectomy (cutting off the tip of the problem root and sealing it from the outside).

Because of the infected nature of the abscess there can be some discomfort or swelling. This will be controlled with antibiotics and anti-inflammatory as needed.

What happens if I don't have a root canal treatment?
If a tooth has an abscess it can firstly cause severe pain, but not always. Some abscesses can be surprisingly painless. Nevertheless, the infection at the root tips can spread into the surrounding bone and soft tissues and cause more serious infections there that could cause swelling and pain. Bacteria also enter the blood stream and can infect other parts of the body. Prosthetic hip and knee joints and heart valves are particularly prone to infection. Patients with other health problems could also be at higher risk.

Root canal treatment is too costly. What alternative dental treatments are there?
Unfortunately, usually the only alternative dental treatment that a dentist can suggest, is to remove the infected tooth. Often teeth requiring root canal treatments are weakened from the large tooth decay cavity and cannot easily be simply extracted. Many require surgical removal. Initially the removal of the tooth may save costs, but in the long term replacing the missing tooth is far more costly than root treating it and restoring it with a suitable restoration, usually a crown. Dentists advise against leaving gaps.

Following Your Root Canal Treatment:
Our dental care clinic wishes to inform our patients of what to expect following root canal treatment. When a tooth is in the process of healing, there are many factors to consider. Every tooth is different, some are more infected than others and some are more difficult to treat. Therefore, there will be some sensitivity and the tooth will require some time to heal.
It can take up to two weeks for a tooth to settle completely. There will be some sensitivity to pressure, in which we advise to avoid the area. Sometimes patients experience swelling in the area, either from infection or traumatization. It is not unusual for a patient to develop more sensitivity or swelling or both in the treated area three to four days following treatment. This increased sensitivity in the tooth root is considered normal. Please continue to take your antibiotics, if prescribed, and pain medications as needed, as they will both assist in the healing process. Our dental surgeons recommend that you to take your antibiotics with food. There is no need to wake up in the middle of the night. Just take them with breakfast, lunch, dinner and a snack at bedtime.

With lower teeth you may experience a stiff jaw and or some soreness upon opening and closing. If this occurs, apply heat and massage the jaw joint area. The freezing injection is given in the joint area and can sometimes cause stiffness. It may take up to a month for the jaw to get back to normal.

PLEASE BE AWARE THAT THERE IS A TEMPORARY FILLING IN PLACE WHICH ONLY LASTS FOR APPROXIMATELY ONE MONTH. Therefore, it is important that you see your dentist to have a permanent restoration placed. This is of extreme importance in order to protect your tooth.

 
Dentures

Patient Instructions and Guidelines :
 
Millions of people face tooth loss every year, but luckily there are several options in modern day dentistry to replace the teeth with treatments dental implants, fixed bridges and dentures.
People are advised dentures for the missing teeth by the dentists for the following reasons:
  • Teeth adjacent to the gap (where the tooth was lost) begin to shift (move) towards the gap and cause functional bite problems.
  • Missing teeth can result in bone loss and shrinkage of your jawbone over time.
  • Missing teeth can cause your facial muscles to sag (due to lack of support) and make you look much older than you are.
The dentists use two main types of dentures to replace missing tooth, full dentures which replace a full set of either upper or lower teeth and the dental surgeons also use partial dentures which only replace a few missing teeth.
 
Denture Hygiene
Dentists advise all the patients to always rinse the dentures thoroughly under running water before any of the following:
  • Soak dentures daily with an immersion cleaner following the manufacturer's direction.
  • Clean your dentures a second time each day using a denture cleaning paste and a denture brush. Do not use toothpaste.
  • Scrub your dentures weekly with a thick mix of cleaner containing bleach. Rinse thoroughly when completed.
  • If heavy calculus (tartar) deposits form on your denture - soak overnight in white vinegar, brush area carefully in the morning and rinse thoroughly. This procedure may be used on partial dentures.

When handling your dentures do so over a sink filled with water. This will cushion them should they slip and fall. When your dentures are being cleaned, rinse your mouth thoroughly with water. Brush your upper and lower jaw ridges and tongue with a soft bristle tooth brush. Finger massaging of the ridges is also helpful.

 

Sore Spots
Expect your new dentures to cause some sore spots in the beginning. These will be readily relieved by your dentist during the first few adjustment appointments. As time goes by changes in your jaws or your occlusion (bite) may also cause sore spots which can just as readily be relieved.

Speech
Practice, practice, practice!!! Be patient and your speech will improve. Practice reading aloud and alone, as if someone until your speech is clean and natural.

Dislodging
Sudden bursts of air such as coughing, sneezing and laughing; very wide opening such as yawning; or extreme tongue movements will dislodge even the best dentures. Avoid these movements or place your hand over your mouth.

Adhesives
Use denture pastes and powders only as directed by your dentist.

General Advice

  • Be sure to leave your dentures out overnight or for at least 2 hours each day. Dentures should be stored in a covered container with the cleaning solution when not in the mouth.
  • Generally the upper denture will be better than the lower. One must learn how to use and retain the lower denture.
  • In the beginning, do alone, as much talking, reading aloud and eating as possible. This will allow you to avoid embarrassing moments as you learn to use your new dentures.
  • I Avoid very hot foods, your temperature sensitivity will be changed by the dentures covering your jaws.
  • In time, due to continued resorption of the jaw, your dentures may require relining.
  • Have your dentures, mouth, and adjacent structures checked by your dentist at least once a year. The needs and nature of each patient vary, therefore, check with your dentist about anything not covered here.
Dentures can be regarded as the best dentistry treatment option for the people who can not choose dental implants or the orthodontic treatment for some reasons like age or health conditions. Dentures can also fulfill the need of a cosmetic dental treatment giving the patient a better appearance, better look and the confidence back again.
 
 Extractions

 
A Close Look At Wisdom Teeth

Sometimes toothache can be so severe or a tooth so broken down, that you may want just to rush to your dentist and have the tooth extraction process done. Most of the times it is the wisdom teeth that cause problems and so need the removal treatment.

Sometimes the wisdom teeth can be pulled by the dentists, although in most cases they need to be cut out by a qualified dental surgeon. When the time comes to have your wisdom teeth extracted, you'll need to go to an dental surgeon and have a consultation first surgeon

During your consultation from the dental clinic, your dentist takes a few x-rays that will let the surgeon know how bad your wisdom teeth are. The surgeon will go over the result process, take a look at your teeth, and then tell treatment options you have. If he is going for a process like removal treatment for your teeth, you'll have the option of using local anesthesia or going with IV sedation. A removal treatment with IV sedation is the preferred way to have wisdom teeth extracted, as you'll be so relaxed you won't know how the whole teeth extraction process is going on. If you decide to just use local anesthesia, which is numbing, you'll be fully aware of the extraction procedure.

Depending on the shape, size, and the formation of the wisdom teeth, the tooth extraction treatment can vary from easy to hard. If the root tips have managed to wrap themselves around the bone, the removal process can be very time consuming and quite painful. Once the extractions have been completed, there is normally little to no swelling involved. Your dentist will prescribe you some pain medicine, which you should use as soon as you arrive home. If you are going to use IV sedation, you'll need to someone to accompany you, as you won't be able to drive home. Having a tooth extraction treatment is the same as having an operation and, because of this, you must look after the area to speed healing and to reduce the risk of infection. Here are some precautionary measures advised by our dental surgeons:

 
  • For the first 24 hours, try to avoid eating hot food, don't smoke, and don't drink any alcohol and try not to disturb any blood clot which might have formed in the tooth.

  • Don't rinse your mouth for six hours after extraction. After that, rinse gently with warm salty water - half a teaspoon of salt in a glass of water is enough.

  • Brush your teeth as normal to keep your mouth as clean as possible.

  • Sometimes the patients may feel some small pieces of bone work way out of the socket - don't worry, this is perfectly normal.

  • There may be some swelling in the tooth after extraction treatment and a bit of discomfort in the first two to three days. You can take some ordinary painkillers like- aspirin, ibuprofen or paracetamol if needed.

  • If you feel pain immediately after the tooth removal, it might be where the blood clot has broken down leaving an empty hole in the gum. This is called a 'dry socket' and will need to be looked at by your dentist. In such cases, go back to the dentist and the dentist will pack the wound to ease your discomfort.

What is a crown?
Crowns are dental restoration treatments that fully cover, or 'cap' the natural tooth in order to restore the tooth's shape or improve its strength when it has been heavily filled or damaged in an accident. Crowns can also be used to improve the appearance of teeth that are stained, spaced or slightly out of position where veneers are considered unsuitable.

What are they made from?
Crowns can be made from metals such as stainless steel or gold, composite resin, dental porcelain or a combination of porcelain and metal alloy.

How long do these restorations last?
Our dental care clinic believes that crowns are one of the strongest restorations for heavily filled and root treated teeth. The dentists working with us use materials of the highest quality and contract with dental technicians that we know produce quality laboratory work. When the dentist places a crown or bridge we expect it to provide decades of dental restoration. However, for the restoration to become a reality you need to take proper care of your teeth and gums. While crowns are very durable, plaque bacteria produce acids and toxins that can attack the tooth under the crown and the gums around the crown causing it to failprematurely.

What if I don't crown a heavily filled or root treated tooth?
All fillings weaken teeth to some degree. While teeth restorations done with newer bonded composite resin ("white") have a lesser impact than silver amalgam, any filled tooth may develop cracks over a period of time. Root treated teeth especially become brittle due to the absence of the natural pulpal fluids inside the tooth. As the stress cracks at the base of the cusps get bigger the tooth may become painful to bite on, the nerve inside the tooth may be so affected that a root treatment is required or part of the tooth may break away. Sometimes the tooth may split so badly that thetooth must be removed and replaced artificially.

Steps in making crowns

Root treated teeth with gum recession. Dark root necks are visible above the old crowns. The gums above the crowns are red and swollen from the ill-fitting crowns.

In the restoration process, the old crowns removed and the teeth prepared for new crowns. Gold posts placed down the root canals and cores build up the damaged teeth. Impressions are made and the teeth protected with plastic temporary crowns while the final crowns are manufactured.

The new all-ceramic crowns are tried in and checked for fit, colour and shape. The new crowns are then cemented in place with a permanent cement. With the completion of this restoration process a very natural look is achieved with healthy gums at the crown margins.

Not everyone can have dental implants, so talk to the dentist to help you assess your individual needs.

An implant is an artificial tooth root and can be made from various materials including titanium. Unlike removable dentures or fixed bridges, dental implants are surgically "implanted" under the gum tissue in the jaw bone.

The process will generally start up with surgery to place the anchor of the implant. Depending on the type of implant, the surgery will "implant" the anchor, in some cases with a post already attached that is used to connect the anchor to the replacement tooth. With other implants, a second surgery may be necessary to place the post. Once the implant has bonded with the jaw bone and the gums have healed, an artificial tooth that looks like a natural tooth is made and attached to the implant. Finish the final minute of brushing by focusing on the upper and lower front teeth. Angle the toothbrush where it is comfortable and flexible. Remember to brush the sides facing the tongue as well as the sides facing the lips.

You should start flossing your child's teeth when they touch each other. Back teeth will usually be the ones to touch each other first. You can use a piece of dental floss or a child's dental flosser. Kneel in front of or to the side of your child. Draw an imaginary vertical line through the centre of your face. This mid-line should run through the centre of your eyes, nose and chin. Ideally, the mid-line should also run through your two central incisors thus making the central incisors a mirror image of each other.

Smile Width:
Ideally, a wide smile will show your first molar to first molar tooth. Although if you have a narrow smile you may only see the front six teeth from canine to canine.

Draw an imaginary curve along the bottom of your upper teeth and compare it to an imaginary curve of your lower lip. Ideally, your smile should be curved in order to look younger.

Ideally the width of a tooth should be approximately 80% of the length. However for patients that wear/grind their teeth, this ration is closer to 1:1 as the teeth have shortened and become squarer. This is a way to measure exactly how much tooth has been lost over the years and is often used to rebuild edges of teeth to their ideal proportions and perfect smile.

Embrasure Space:
These are the little triangular like spaces between the tips of the front teeth. Without embrasure spaces, the teeth would look like flat piano keys with no character to them. Ideally, the embrasure spaces should be smallest between the two central incisors and gradually become larger as you examine teeth further into the mouth. An ideal feminine smile tends to have more definite round embrasures.
